VNA Now Hiring Hospice Social Worker

VNA-Hospice-Care-Program-Officially-Accredited-by-the-Accreditation-Commission-for-Health-CareThe Visiting Nurse Association Northwest Indiana is seeking a Hospice Social Worker (full-time) to provide direct supportive services to terminally ill patients and their families at our in-patient Hospice Center and Patient Homes.

The Hospice Social Worker works in conjunction with the Hospice interdisciplinary group (IDT) to provide direct supportive services to terminally ill patients and their families, and to consult with and support other agency personnel .The social worker will provide assessments of psychosocial problems and define goals and interventions to provide excellent end of life care.

The Hospice Social Worker:

  • Is a member of an interdisciplinary team (IDT) and provides social services in accordance with interdisciplinary plan of treatment
  • Participates in the preparation, evaluation, and execution of the interdisciplinary plan of care to assure that the highest quality of care is given to those served
  • Recognizes and utilizes community and family resources to assist the patient and family with end of life care needs
  • Develops the plan of care with the IDT to deal with personal, financial, and environmental difficulties that may interfere with care
  • Assists with funeral planning
  • Keeps abreast of social work trends and knowledge
  • Participates in education both internally and externally to promote professional growth and understanding of hospice
  • Maintains continuity of patient care by liaising with other health care professionals
  • Attends IDT conferences and care plan meetings
  • Arranges for extended care placement or respite placement when necessary

Requirements

  • Demonstrates flexibility with scheduling
  • Must possess a Master's Degree from a school of social work accredited by the Council on Social Work Education
  • Must be licensed by the State of Indiana

Required experience: Minimum of one year in social work, preferably in a healthcare related setting

Preferred education: Master's degree
