Important information related to eligibility for the following health care coverage programs will be presented:
Hoosier Health Wise/Medicaid (Several Different Plans Available) – The Hoosier Healthwise program does not routinely cover men or women over age 19 if there is no disability or pregnancy at the present. This assistance is based on income and assets.
HIP (Healthy Indiana Plan) – The HIP program is available to people with no insurance for the previous 6 months (if employed, may not have refused insurance offered by the employer). Eligibility is income-based, and there is currently a waiting period to get on HIP.
Porter’s Financial Assistance Program – This program is for people who have received care at Porter, have no insurance, and have been denied for all other assistance programs. It is based on family size and income.

People interested in applying for coverage through one of these programs will need to bring the following items to the Eligibility Fair:
- Photo ID
- Social Security Card for all applying for assistance
- Birth Certificate
- Income Information for the last 30 days
